SpEd uses a logbook process to collect data following its SEDS Protocol.

The data sources are the SEDS forms used by SpEd Teachers to document the SpEd process for each child. Completed SEDS forms and support documents are maintained in the student file at each school. Go to SEDS Forms

For Annual Performance Reporting, the logbook is one of the data sources. Discipline, State Assessment, Personnel, Dispute Resolution, and others have to be requested at the time they are needed from their source offices.
